In-home visits are conducted by two members of the care team, your provider, and a medical assistant. Our in-home providers are experienced nurse practitioners and our family advocates are highly-trained medical assistants. Your Nest team may also include family doctors, nurses, and mental health specialists to care for you and your family by phone and video when needed.

If you and your family members are covered through a participating health plan, Nest services are available at no additional cost to you.

No, Nest is a primary care provider and will not replace your OB/GYN. We partner with your specialists to get you and your family the best whole-person, well-coordinated care. 

Your Nest team provides care to parents under the age of 65 and their children, ages 0-18 who live in the same household. We offer primary care, mental and behavioral health care, access to specialists, nutritional support, and more.

Nest in-home visits are conducted by a family provider (nurse practitioner or physician assistant) and a family advocate (medical assistant).To make things easier for your family, Nest will combine appointments to see multiple members of your family in the same visit, with time set aside for each individual. Our appointments typically last between 30 and 60 minutes, depending on the specific needs and size of your family. With not having to travel to a doctor’s office, your family will save time with Nest.

Nest will manage all conditions that are typically cared for by a primary care physician. This includes, but is not limited to, hypertension, heart disease, diabetes, asthma, high cholesterol, thyroid disease, depression, and anxiety. Treatment will include medication management, laboratory assessment, patient education, and individual and family care management.

Our Nest team will make sure you and your family have the comprehensive care you need. We have a team of specialists available to consult with our providers when further clinical expertise is required – all without you having to leave your home. If you do need to be seen by a specialist in person, we can place referrals and to ensure that those visits are scheduled and meet your needs.

Nest can treat many basic, acute care needs that you might go to an urgent care facility for (fever, sore throat, cough, UTI symptoms, minor skin issues, etc). We have nurses available 24/7 to listen to your symptoms and concerns, give advice, and connect you to a doctor or other provider if needed. We can schedule telehealth visits for acute care needs. Because Nest is a primary care provider, we can also provide well visits, preventive care, and treat other chronic conditions (hypertension, heart disease, diabetes, asthma, high cholesterol, thyroid disease, depression, and anxiety).

In the case of an emergency, please call 911 or seek care at your nearest emergency facility as Nest is not equipped to manage emergent or urgent needs in the home or virtually.
